Educational Department and Related Forms

These forms require Adobe Acrobat or Adobe Acrobat Reader to view and print. To print .PDF files, be sure to click on the printer icon on the left of the bar across the top of the Acrobat Reader window; do not use Print from the browser File menu or toolbar.  Use of Acrobat or Acrobat Reader 7.0 or later is recommended.

Some of these PDF forms may be filled in on the computer; these forms are shown with an asterisk below.  Click in the first field where you want to enter data, and then type in your entry.  Use the tab key or your mouse to move from field to field to continue typing in the information.  If you are using Acrobat Reader to fill in the forms, you MUST PRINT the completed form prior to closing the document, and then send the printed copy to the appropriate recipient.  Acrobat Reader has no facility to save your data after it has been entered on the form, and once you close the document, the data is lost.  If you use the full version of Acrobat, you can save your completed form electronically.

Please be aware that some forms require a signature or other hand written information prior to sending to the intended recipient(s).  Please check the forms carefully after printing and prior to transmittal to ensure all entries have been completed as required.

As forms are updated, they will be made to allow you to complete them on your computer.

USPS HQ has a toll free fax number that can be used for ordering EdDept materials.  The number is 1-888-304-0813.  The original fax number is also available: 1-919-836-0813.  Both numbers are now included in the latest edition of the EdDept catalog.

Note that some older forms may have references to older editions of the USPS Operations Manual. The older editions had most of the relevant Educational Department contents in chapter 12; the latest edition of the USPS Operations Manual now has these contents in chapter 6. (Apr 06)

        SOF* - Standard Order Form  (Sep 15)   Click for Help.   Only for use by those who cannot use the On-Line Catalog and Shopping Cart.
        ED-33* - Examination Order and Sight Folder Submission Form (Oct 10)   Only for use by those who cannot use the On-Line Exam Ordering process for exams. This form MAY be used for sight folder submissions to HQ.
        eED-33 - On-line Sight Folder information form (Jul 10). Used to prepare a printed form that must be submitted with the sight folders to USPS Headquarters.
