Yearly Merit Mark Activity Reports are due by October 15. Please help us collect information on your efforts and participation in the Power Squadron this year. List all activities/positions even though these may also be reported by Committee Chairs or other persons for you. Information on this form may be submitted to the commander at any time during the year, but all information for the year should be in by late November so his report for the year can be filed with USPS.

Be as descriptive as possible (position held, activity name, type of work done, dates(s), number of hours volunteered, #of calls made, # classes taught, # events worked on, # tests graded, etc...)

Members: Print out this form and turn in completed reports to Squadron Commander for consideration.
