Members: Report your current year activities to our Merit Mark chairperson by 20 October.

You may report your activities for the current year by downloading the Merit Mark Form, filling in the information and attaching it to an email addressed to our Merit Mark Chairperson.

Procedure:

1. Click on the following link to download the form. Click Here

2. Download the form to your computer.

3. Enter the information.

4. When complete, attach the form to an email and send it to D/C Zambroski <gzambros@rochester.rr.com>

Note: The form must be opened with MS Word.
