There are three major forms used by
membership chairs. All forms contain explanations on how to complete the form: (Note: The four types of membership, the procedure to allow
someone to become a member and the requirements for membership are located in the National Bylaws Article 3)
New Member Form (MEMCOM1)
This form is the New Member Application (MEMCOM1)
which every new member to the United States Power Squadrons® must complete and sign. It is processed
according to Article 3 of the Bylaws and the form is kept according to the Operation Manual 5.31 or according to the Standing Rules of the squadron.
The data from MEMCOM1 is entered via Member Stuff/New Member Processing on the www.usps.org main page.
For new members taking advantage of the trial membership, use the "ABC3 Student Trial Membership" option.
For a new dues paying member use "Add New Member Records - Full Active" and send the dues to the squadron treasurer. Further details can be found
under the National Membership Committee (MEMCOM) website "How to/Tools".
Transfer Form (HQ110)
This form is used to transfer a member from one squadron to another. The top three parts of (HQ110) are to be completed by the transferring member
and sent to the Membership Chair. Part 4 can be completed by the secretary or the membership chair. (If completed by the membership chair, the
secretary should be notified.) The form is then forwarded to Headquarters for passing to the releasing squadron. It is the responsibility of the
releasing squadron secretary to ensure that the member transferring does not have any outstanding material or monies due to the releasing squadron
before sending the form back to Headquarters. Further details can be found under MEMCOM's website "How to/Tools".
Reinstatement Form (HQ103)
This form is used to reinstate a member who has allowed his membership to lapse more than 2 years from your squadron. If a member is on the squadron
Dues Status Report, found at www.usps.org/dues/reports, he/she can simply
renew by using the invoice number and his/her certificate number to renew online.
If a member from another squadron (no matter how long his/her membership has lapsed) or a previous squadron member is not on the Dues Status Report, this
form is used. Again the instructions are on the form. The form is emailed to Headquarters and the dues amount is paid to the squadron treasurer.
