HQ 102 - Personnel Change Report  -  Use for Address Change - Name Change - Report a Deceased Member - Report a Resignation.

Want to send in the change faster?  Please note:  Recently, support was added at headquarters to correctly handle electronic HQ-102 forms created via the Data Base Management Program (DB2000).  This support now gives DB2000 users the ability to process name changes, resignations, and death notifications in a paperless manner.  Access to the HQ-102 form dialog is found on the Forms menu.   The help menu of the form dialog describes fully how to use the form.
