Inflatible Life Jacket Accountability
Worksheet Helper For District Chairs
At the end of 2011, all incoming reports from Squadrons were compiled and
all the 2011 figures
for in possession or Lost/Damaged/Destroyed were entered
into a data base to get the accountability
for Squadrons for the year.
The compilation resulted in the following worksheets being made available
for squadrons for the 2012 accountability year. This accountability sheet is
easy to interpret and adding information will automatically total up your form.
Open your District Worksheet and you will see all the Squadrons for that District. You are only concerned with your squadron figures.
Column G is your Squadron name
Column H is the number of Inflatable Life Jackets in your possession as of the
report date NLT 15 November 2012.
Column I is the number of life jackets possibly Lost/Damaged/Destroyed in 2012
(Note these on the VSC-5 report)
Column J just reflects what was reported or NOT reported in year 2011.

in your Inflatable Life Jacket Report for 2012.

Note: The pdf form is a fillable form and takes a minute to load and then you
can just type in your information. When you save the pdf to your Windows
computer, browse to the file and right click it and pick 'send to' and then
'mail recipient.' It should open your mail program and you can mail it
direct to Stf/C Lee R. Chasse at the email chasselr@comcasat.net
Just click on your District number below to download and save to your desktop or to open and work with the form and then save to an area of your choice. (These reports may take a minute or two to download from the USPS Server)
