[D-plan-net] Survey date

It is nice to know that people actually read the posts to the List. It has been brought to my attention that I have conflicting completion dates for the survey. Please use the January 15 date which was the date in the initial post.  The incorrect date was due to my error after too much time trying to keep up with Grandkids over the Holidays.  I apologize for any inconvenience.
