[D28] US Power Squadron D/28 Certifiers' Class for BOC (Boat Operator's Certificate)

I downloaded this form and was filling it out, but found that it was
intended to be used for a third party to nominate an instructor, not
to nominate yourself. Do we get you guys to nominate us if we're
interested?
