Bob,

You are correct and I did find ''Chapman Award'' the same way you did.  However, it's not a file that can be edited or converted to a workable word document.  That leaves two ways to fill out the forms after one prints them out.  One way is hand scribe each form, which leaves a lot to be desired considering the age of us making out the form. The other way is to use a typewriter to fill in the various fields.  Does anyone know what a typewriter is anymore, never mind where to obtain one?

We can fill out other education forms, transmit them or just print them out.  Can't do that with the Chapman nomination forms.

  Does anyone have a PDF template so the Chapman Award forms can be filled out on a PC? I can convert a word document to a PDF file, but can't convert a PDF file to word in the correct format.
